New Families

1. Fill out an online application by clicking the link below.

2. Once approved, you will go to Parentsweb and fill out enrollment papers and pay the required, Non-Refundable fee of $175 per Child.

  • Our District Code is: SMY-OK

3. Submit Documents:

  • Birth Certificate for Each Child

  • Sacramental Records for Each Child (If Catholic)  

  • Immunization of Records for Each Child (Kinder must have current shot records; Students entering the 7th grade must have their Tdap booster)

  • Vision Screening & Comprehensive Eye Exam Certification (Applies only to Kindergarten, First Grade, And Third Grade.)

  • Custodial Agreement (If the child does not live with both parents, please provide court documents of custody/guardianship.)

Returning Families

1. You will need a Parentsweb account. If you do not have one, click create account on the Parentsweb site or call the office for an email link.

2. Once logged, in click family information (on your left.) Then select re-enrollment/enrollment, click and follow instructions.

Additional Information

All new applicants are required to submit the following information to be considered for acceptance at Saint Mary's Catholic School:

Students entering Grades 1-8 must provide copies of report cards for the past two years, academic test results.


If applicable, please bring a copy of any Individualized Education Plans (IEP), Individualized Service Plans (ISP), or 504 Plans and make an appointment with the principal to make sure we can accommodate those. 

Students entering grades 1-8 may be required to have an admission interview. Families will be called to schedule an interview if this is necessary. Students entering Kindergarten will be invited to screening sessions if necessary.
